About the Role
Om Carehome is seeking a dedicated and compassionate Care Coordinator to join our team. Requirements
Previous experience in care coordination, healthcare, or social work is preferred.
Strong understanding of care planning and client-centered service delivery.
Excellent organizational and time-management skills.
Strong interpersonal and communication skills to liaise effectively with clients, families, and carers.
Proficiency in maintaining detailed care records and documentation.
Ability to work in a fast-paced environment and handle emergencies with professionalism.
Familiarity with regulatory compliance in the home care industry.
Compassionate, patient, and dedicated to improving clients' well-being.
Proficiency in using care management software is an advantage.
Relevant qualifications in healthcare, social care, or related fields are desirable.
